Nurse Practitioner

Collaborate with some of the country’s most prestigious health systems and PCPs as an embedded onsite team member in their practice or at one of our Wellness Centers.

Expertly use preventative health screening tools (FloChec, Spirometer) within a clinical setting‬‬.

Serve as an educator and trusted advisor to the geriatric patient population, as part of annual wellness visits and performance of health assessments.

Assist in closing quality care gaps (screening and labs) and enhance overall care coordination.

Qualifications

• Current Nurse Practitioner Certification is required
• Experience in primary care / adult / geriatric care preferred
• EMR skills preferred
